I would love to give my students a good end of year survey. Does anyone have one they like? If so, I would love to see it. If not, do you have ideas for questions that would be helpful on a student survey? I plan to make mine on a GDoc Form.

Here is something I found on the internet and was planning on revising. See if you find it useful.
The second one is just a compilation of things I found that I wanted to remember for when I actually took the time to write one of my own.

I used to give oneveryone in my calculations class I called ” the aftermath”. The kids always loved the name which helped in making them willing . 🙂
The format was good, even if topics were not the same.

I love the title!
I couldn’t open the page. It said I needed to sign in. I LOVE the title however!
One thought I’ve been pondering: I might ask my kids to write a letter to themselves at the beginning of the year (going back in time) or to students who are taking the class next year. I figure they’ll learn a lot and I’ll learn a lot from their responses, if they are thoughtful.
Funny! I had a similar thought. I was thinking about having them write a letter to next years students giving them advice about my class!
Here is a link (sorry couldn’t figure out how to hyperlink) (https://docs.google.com/forms/d/1RBCdafxSMgeNeRwOPX0EiY7w8_FXAwBlVVea0GInXhA/viewform?usp=send_form)
to the end of year survey I give my students (stolen in large part from Matt Vaudrey!) I’ve found its given me some great information to change what I do each year!
Thank you!
mathequalslove’s Start, Change, Stop, Continue is a nice simple survey I like to use.
